Social Care Network (SCN) Systems Administrator

Project Summary:

The New York State Department of Health has established Social Care Networks (SCNs) as part of the 1115 Waiver Demonstration Amendment, and Forward Leading IPA (FLIPA) is the SCN Lead for the Finger Lakes Region. This region is comprised of 14 counties, including Allegany, Cayuga, Chemung, Genesee, Livingston, Monroe, Ontario, Orleans, Schuyler, Seneca, Steuben, Wayne, Wyoming, and Yates. The SCN’s aim is to enhance the delivery of social care services to Medicaid members by coordinating efforts among community-based organizations (CBOs) and other health care partners to create a more resilient, flexible, and accessible social care system that reduces health disparities and advances health equity. The SCN will collaborate with CBOs and other health care partners, leveraging shared data and technology to coordinate social care services for Medicaid members to improve access, ensure reliable and timely referrals, streamline and track navigation and completed referrals in closed loop systems and enhance collaboration between social care service providers and other regional partners.

Organizational Overview:

The Forward Leading Independent Provider Association (FLIPA) is a nonprofit membership association of safety net providers working in partnership to provide the highest quality healthcare to the most vulnerable populations in Upstate New York since 2017. FLIPA is renowned for its commitment to integrating primary care, behavioral health and social care. Our growing membership includes a combination of federally qualified health centers (FQHCs), behavioral health providers, and a rural health network across more than 27 counties.

Job Summary:

The SCN Systems Administrator is responsible for managing IT solutions and platforms to ensure successful implementation and operation of the SCN. This role is responsible for management of required timelines/deliverables of closed loop referral vendor(s) and related IT and data solution vendors. The SCN Systems Administrator works with FLIPA SCN leadership, partners, and identified IT/data solution vendors to meet project timelines and deliverables, manage relationships, and communicate key performance indicators. This role supports onboarding to closed loop referral system, compliance reporting including user audit and offboarding procedures, monitoring security and associated risks, troubleshooting of issues/concerns, and coordinates associated remediation and training support for performance improvement. This position is grant-funded through March 2027.

Duties/ Responsibilities:

  •  In collaboration with SCN Project Director and FLIPA Leadership, develop a project workplan that ensures platform(s)/IT solution timelines and deliverables will be met and all key performance indicators communicated as required
  • Direct relationship management with closed loop referral vendor(s)/IT and data solution vendor(s) and SCN Leadership to ensure adherence to timelines/deliverables on application implementation.
  • Develop and maintain platform/IT timely onboarding and offboarding procedures for all SCN, partner, and CBO staff.
  • Works with Compliance Officer to develop user audit procedures and reporting.
  • Serve as the Super User for software/IT use cases and workflows in conjunction with document management of system(s)
  • Communicate regular reporting on user access, connectivity, related key performance indicators, progress and concerns to relevant team members
  • Participate in development of training and educational materials about the platform and SCN IT solution(s)
  •  Conduct training and remediation support for organizations as needed to optimize use of the SCN platform(s)/IT solution(s)
  • Maintain liaison with software vendors for product issue escalation, product enhancement requests, user groups, and product news such as upcoming release features and schedules
  • Investigate system problems and coordinates fixes or functional enhancements with software/IT vendors.
  • Assist network constituents with alternative system workflows if system functionality cannot be fixed or enhanced
  • Ensure project activities are assigned to appropriate resources for completion
  •  Monitor project progress and proactively work to identify and mitigate risks and barriers that may impact implementation of project activities and goals
  • Collaborate as appropriate to establish and manage resource library for systems, processes, and procedures
  • Other duties as assigned

Education & Experience:

  • Bachelor’s in computer information systems or related field or equivalent experience required
  • Minimum of 3-5 years of experience in a systems administrator role, preferably in a healthcare or social services environment
  • Strong project management or related experience required
  • Prior experience managing closed-loop referral platforms preferred
  • Experience within the New York State health care system (population health, behavioral health, Federally Qualified Health Centers, or similar) preferred

  • Strong interpersonal skills, independently resourceful and thrives in a fully remote environment
  • Fundamental understanding of data sharing/data integration/interoperability, between disparate systems
  • Understanding of healthcare, health-related social needs (HRSN) terminology, including screenings, cases, types of interventions, and social needs assessments
  • Comprehensive understanding of the mechanics of healthcare programs, including case management, care coordination, and patient engagement strategies
  • Proven ability to complete projects according to defined timelines and objectives
  • Proficient with MS Office and various business software

Salary Range:

  • Salary is commensurate to education and experience with range of $80-100k.

Additional Info:

  • This position will be remote with regular travel expected for on-site meetings or training across the Finger Lakes Region and New York State
