At Forward Leading IPA, we understand that a healthy community starts with more than just healthcare. Our Social Care Network (SCN) connects individuals and families to vital resources that address the social factors affecting health, from housing and employment to food and transportation.

What is the Social Care Network? The Social Care Network is a collaborative effort, bringing together health centers, behavioral health providers, and community-based organizations (CBOs) to create an integrated delivery system that tackles the root causes of health disparities. By linking health services with social care, we aim to improve the overall well-being of  individuals on Medicaid across the Finger Lakes region.

We are committed to an equitable, accessible, and integrated healthcare system that leads to better health outcomes. With a focus on meeting the social care needs of our community, we work hand-in-hand with partners to ensure that every individual has access to the essential resources they need.

How We Work

The Social Care Network collaborates with community partners to identify individuals in need, connect them to appropriate resources, and provide ongoing support. We believe in a transparent and sustainable model that focuses on empowering our community by investing directly in capacity building for our partners.
