
Concerns & Grievances
WeLinkCare is committed to quality services and supporting the health and safety of Medicaid Members and their families. Members or anyone unhappy with We Link Care’s services, programs or providers can file a concern. Types of concerns may include:
- violation of rights
- issues accessing or receiving services
- dissatisfaction with the quality of care or services
- the lack of options or issues with providers
- unhappiness with program eligibility
Concerns should be filed urgently. If you wish to talk in your language or need help with filing, we can help at no cost. A concern filed for or by a member will not risk their participation or eligibility for the program. To file your concern, you can:
- call 888 538-2958 (WeLinkCare Concern Hotline)
- submit the concern using the form below
- send an email to concern@forwardleadingipa.org
- tell concerns directly to a SCN service provider
Concerns should include:
- name, address, phone number of the person submitting the concern
- you may submit a concern anonymously, however, you will not know the outcome or have direct follow up
- if acting on behalf of the member, include the member’s name and other known identifiers
- the location, date, and a description of the concern
All concerns are reviewed, documented, and responded to in a timely manner. Concerns are addressed by reviewing the details, speaking directly with the member or when applicable, the person who filed, and sharing the resolution or outcome with those who need to know.
