The Social Care Network (SCN) program offers services that can meet your needs if you qualify. A Social Care Network Navigator will talk with you about your needs. They will see if you qualify and identify what services may be available to you.

How Medicaid Members Can Access Services

Through Social Care Networks

 

As a Medicaid member, you can connect to services through your local Social Care Network (SCN). After you get screened, a Navigator can help you get things like housing and utility supports, meals, and transportation.

  • WeLinkCare is the name of the Social Care Network in the Finger Lakes region.
  • WeLinkCare is supported by Forward Leading IPA (FLIPA) who is NYS’ Social Care Network Lead Entity.
  • WeLinkCare is a group of agencies committed and approved to provide Health Related Services Needs (HRSN) services to qualified Medicaid Members.
  • WeLinkCare uses the FindHelp website to complete screenings, eligibility assessments, and connect Members to HRSNs.

Screening: Answer questions about your needs to see what help you may qualify for

Navigation: Get help finding services in the community Social care services. If you qualify, you may get help with:

  • Food and nutrition, such as meals delivered to your home or cooking supplies

  • Housing, such as finding a place to live or making changes to your home to keep it safe.

  •  Transportation, such as rides to housing or childcare appointments

You may be able to get SCN services if you have Medicaid Managed Care.

Medicaid Managed Care is a type of New York State Medicaid. You must also have certain health conditions to qualify. For example, you may qualify if you:

  • Have a mental health condition or substance use challenge and need food or housing to stay health

  • Have a disability and need support at home or in the community

  • Are pregnant or recently had a baby and need help with food or safe housing

  • Recently left jail or prison and have a long-term health problem or a substance use problem

  • Are a child or young person with health needs like asthma or diabetes and need support at home

  • Have frequent visits to the emergency room or hospital

  • Are enrolled in a New York State Health Home
